Cancer Patient Pathway Coordinator
is responsible for being the patient’s advocate during their entire
pathway of care, at all stages of their disease from suspicion of cancer to diagnosis and subsequent
treatment within HMC. Activities include: providing counseling, education, support and advice to patients
and their families and carers.
In addition to patient duties the post has a quality and service improvement dimension. The post-holder will
be required to apply service improvement tools and techniques to improve the patient pathway in line with
the recommendations from the National Cancer Strategy, and to contribute to clinical audit.
The post holder must critically think in variable situations within clearly defined policies, principles, and
leadership direction with multiple healthcare stakeholders across the care delivery system.
Given the interface demanded by this role with both clinicians and patients, it is essential that the
post-holder is a clinical professional, from a nursing, medical or allied healthcare professional background,
possessing knowledge of the clinical pathways, medical terminology, and multidisciplinary clinical providers
to which they are assigned and experience of coordinating the related clinical requirements of the patient
pathway.
Principal Accountabilities
• Act as the patient’s key advocate and coordinator through their entire pathway of care for the tumor group
to which they are assigned
• Facilitate timely, effective, quality diagnosis and treatment and the best possible experience for patients
with suspected or diagnosed cancer, their families and carers
• Counsel, educate, support and advise patients with cancer, their families and carers on possible treatment
plans, approaches and lifestyle adaptation
• Apply service improvement tools and techniques to improve the patient pathway in line with the
recommendations from the National Cancer Strategy
• Lead and contribute to specialty clinical audit Communication
Use advanced communication skills to:
• Act on behalf of the patient and in the co-ordination of the patient pathway for patient’s referred to the
multi-disciplinary team (MDT).
• Ensure that findings from assessment and care plans are discussed and agreed with the patient
Coordination and Facilitation
Use advanced coordination and facilitation skills to:
• Coordinate the assessment of the patient’s clinical and psychological needs
• Facilitate inter-professional, hospital-wide and cross-sector liaison between clinicians at HMC and in
primary care to ensure continuity of care and a seamless service for the patient.
Clinical
Use clinical expertise to:
• Attend and contribute to the MDT discussion and decision about the patient’s plan of care (or where
necessary contribute to a cross-cutting MDT such as palliative care).
Education:
Bachelor’s degree in Nursing,
an allied health profession, or the equivalent.
A medical degree (M.D./D.O.) also meets the minimum education requirement.
Diploma in Nursing with substantial oncology experience (more than five years)
Preferred:
Master’s degree in Nursing, an allied health profession, or the equivalent.
