> FOR SURGEONS ONLY: Copy of the Surgical Book
                                          
> FOR SPECIALIST DOCTORS > Log List of the Surgical Procedures 

REQUIREMENTS:

  1.  Updated CV with Picture
  2. CHED ( red ribbon valid)
  3. TOR, RLE and clinical hours certificate
  4. PRC License(front & back) PRC Board Cert, PRC Rating, Good standing Certificate 93mos valid)
  5. Diploma ( college, high school)
  6. Form 137
  7. photo with white background
  8. BLS certificate
  9. HAAD forms
