Position Summary
Responsible for the general processing of all medical records created in the hospital and quality coding of
diseases, operations and procedures.

Principal Accountabilities
- Assemble discharged patient records in standard order.
- checks and analyzes records for accuracy, adequacy and conisistency of documentation
- Assigns appropriate codes for each diagnosis, operation and procedure based on the international
classification of diseases and procedures.
- Prepares deficiency slips for incomplete records
- Collates generated abstracts from medical records and inputs information into the computer
- Records abstracts on discharge list
- Prepare and files out guides for the records which are given to transcription section.
- Answers request for retrieval of medical records and assist in data gathering for research, studies, and
statistics.
- Account for daily discharges and performs audit as necessary
- Perform other related functions.
Minimum: 3 year experience with proficiency in International coding principles (ICD 9/10), medical
terrminology and human anatomy
