Job Description and Requirements

  4. CHED (Commission on Higher Education)
  5. TOR, RLE and clinical hours certificate.
  6. PRC license, PRC rate certificate, PRC rating and PRC good standing good certificate.
  7. Diploma (College and High School)
  8. Form 137.
  9. Passport size photo with white background.
  10. MUST BE A HAAD Passer.
  11. HAAD forms (Authorization, Declaration of Applicant, and Staff Data form)
  12. BLS (Basic Life Support) certificate.
