1 DFA Authenticated (Red Ribbon) College Diploma (School Credential - Diploma, TOR, RLE, etc)
2 Employment Certificates
3 Certified True Copy of PRC License
4 Good Standing from PRC
5 Passport valid for 2 yrs (Original)
6 Passport Size Photos in White Background
7 Training Certificates
8 High School Diploma
9 Detailed and Updated CV
10 Pearson Vue Pass (HAAD Exam Registration Number)
11 Board Certificate
12 Basic Life Support Certificate (for Nurses)
13 Certificate of Name Change (Marriage Certificate, Affidavit, Etc) as applicable
