 JOB SUMMARY
- The provision of a timely, comprehensive assessment (medical, social, functional and psychological) and treatment service for older people with complex health needs
- Maintaining close working relationship with Old Age Psychiatrists and palliative care services.
- Establishing an effective, responsive and safe acute hospital admission
- Provision of improved access to assessment which enables frail older people, and those with long term conditions, to be seen and treated closer to home in an environment most appropriate to their needs
- Enhancing the management of patients within primary care, and actively managing the demand for secondary care services ensuring patients have speedy access to appropriate comprehensive assessment and treatment
- Supporting the wider community health and social care teams by providing expert advice, information and assessment coordinating care to meet the changing needs of patients
- Developing and maintaining effective working relationships with consultant colleagues working within the Trust
- Supporting the development of further patient pathways in conjunction with commissioners e.g. DVT and cellulitis pathways
- Demonstrating improved patient and carer experience through auditing quality standards, outcome data and patient and carer feedback on quality and satisfaction, as an integral part of ongoing performance and quality improvement
- Demonstrating integrated partnership working with intermediate/social care and the third sector to promote independence and prevent admission to long term care
- Demonstrating improved patient outcomes and value for money by utilising this approach
- Developing agreed models of care and pathways for older people with complex needs including co-morbidity of dementia
- Supporting patients in their own homes, including care homes

MAIN DUTIES OF THE POST:
CLINICAL
The post holder will provide:

Targeted, evidence-based interventions for patients which:
- Provides safe, effective and timely rehabilitation/re-enablement
- Reduce unnecessary acute hospital attendances, admissions and readmissions
- Reduce premature and/or inappropriate entry to institutional care
- Improve end of life care in partnership with palliative care team
- Clinical case management for patients referred to the Rapid Assessment Community Clinic
- Clinicalcase management of patients
- Active engagement in multidisciplinary team meetings and reviews in primary care, community and domiciliary settings, the Rapid Assessment Community Clinic and community hospital in-patient units
- Leadership in service development
- Audit and research
LEADERSHIP
The post holder will provide:
·        Advice and support for community and primary care practitioners
- Consultant leadership in the management of in-patients
- Supervision and support for the Associate Specialist doctors
- Collaborative engagement with acute, primary care and local authority colleagues
Candidates must have IELTS WITH TOTAL BAND OF 7.5 ACROSS ALL DISCIPLINE
